To be considered for appointment, please complete the Agent Appointment Form in its entirety.
As part of the submission process, you will be required to upload:
- A copy of your current resident (home state) insurance license
- Current Errors & Omissions (E&O) coverage
Upon receipt of your completed information, Managing Director Debra Gould will submit your appointment information directly to our licensing administrator for processing.
If you received an invitation by USPS mail or email, your name and contact information have already been included in our recruitment database.
This Agent Appointment Website is the exclusive method for submitting your information and requesting appointment consideration.
Submission of the form does not constitute approval or appointment. All appointments are subject to licensing verification and final approval.
